Proudly display your laboratory's achievement! To assist
accredited laboratories with prominently displaying their accreditation
certificates, the ICACTL is pleased to offer certificate
plaques. These attractive, walnut plaques include a plexiglass
cover to protect and highlight the ICACTL certificate.
An excellent format for showcasing your laboratory's accreditation
to all patients and visitors to your facility!

Accredited laboratories automatically receive certificates from the ICACTL for each testing area in which they are granted accreditation [i.e.: Coronary Calcium Scoring, Cardiovascular CTA, Sinus and Temporal Bone, etc.]. Ordering
certificate plaques is optional. The cost of the certificate
plaque is $40 each. Applicant laboratories that choose to order
certificate plaques prior to receiving their accreditation decision
will receive their certificates placed within the plaques, ready
to hang on the laboratory's wall.
To order a plaque for your accreditation certificate, simply
fill out and mail in the online Certificate
