Accessing the Accreditation Application
ONLINE ACCREDITATION APPLICATION
(Laboratories/facilities applying for accreditation in Vascular Testing, Echocardiography, Nuclear/PET, MRI and/or CT)
Access to Online Accreditation is free of charge. Applicant laboratories/facilities are only charged the accreditation fee, due at the time of application submission. Accreditation fees vary by modality. Complete details about accreditation fees and access to the application fee calculator, are found on each division website, in the Seeking»Accreditation Fee section.
ACCREDITATION APPLICATION
(Facilities applying through ICACSF)
Access to the Accreditation Application is free of charge. Applicant facilities are only charged the accreditation fee, due at the time of application submission. The facility will be sent an e-mail with a link to the website to download the application and given a user ID and password.
Additional Fees
Additional fees, based upon size and complexity, apply to eligible facilities with multiple sites (fixed and/or mobile). Please visit the Seeking»Accreditation Fee section on the division websites for details about the multiple site fees.
